	Change of Officer and Advisor Update Form Description Fill Out This document is the contact list for all executive members of your organization as well as the faculty & chapter advisors. Please complete this form in its entirety listing only the offices/ officers that have changed. Should you have any questions please contact the Office of Fraternity & Sorority Life at (704) 687-0360 PHILANTHROPY Efforts Monthly Report Description Fill Out Use this form to report your PHILANTHROPY efforts for the previous month. To report your SERVICE hours, please return to the forms page and use the "Service Efforts Monthly Report" form. .::Remember ::. There's a difference between service and philanthropy Philanthropy - Philanthropic projects are events held with the intent to raise money and/or collect items for donation to a community entity Service - Community service projects include the time spent by chapter members volunteering in the community. 	Roster Update Form Description Fill Out Use this form to change the membership status of members with the Office of Fraternity & Sorority Life OFSL Event Supplies Request form Description Fill Out To request supplies from the Office of Fraternity and Sorority life , please complete and submit this form at least 72 hours before your event to ensure availability. Please be advised all inventory supply forms must be submitted no later than 42 hours prior to the start of the event in question. All forms submitted less than 42 hours prior to the event start time are not guaranteed processing consideration. Any forms submitted less than 24 hours prior to event start time are subject to a $25.00 late processing fee. Scholarship Applicant Resume Description Fill Out Please provide a resume that addresses their academic major, credit hours earned, GPA, extracurricular activities, fraternity involvement, work history, and personal goals. Applicant Scholarship Reference Letter Description Fill Out Please upload a letter of reference for the scholarship applicant.

Greek Village Form

	Greek Village Event Registration and Space Reservation Form - Event with Alcohol Description Fill Out The completion of this form does not constitute permission to host an event or reserve a space. Approval must be received from the Office of Greek Life and the Department of Housing and Residence Life before an event can commence. The contact person will receive email notification regarding the status of their request. IMPORTANT NOTE: The Greek Village Residence Coordinator must receive this form at least two (2) weeks in advance of the desired start date of the event. Include the UNC Charlotte Acknowledgment of Responsibility for Service of Alcoholic Beverages with the submission of this form.
